This keynote presentation will focus on an overview of Neisseria gonorrhoeae (NG) infections in Africa, particularly the epidemiology of NG and how it has evolved over time. This will highlight that the burden of NG in Africa largely affects adolescent girls and young women and their sexual partners who are generally order men. This picture has been historical, however; in the past decade or two, the continent has witnessed a rise in the burden of NG among MSM, which has been directly proportional to the increase in the population of MSM in the region. This highlights that the drivers of transmission in Africa are two-fold; heterosexual and homosexual transmission hence public health effort need to focus on controlling both drivers. Alike other parts of the world, Africa has also witnessed a rise in the burden of NG associated with increased access to antiretroviral medication for treatment and pre-exposure prophylaxis. Data will be presented to illustrate this. The presentation will also highlight the NG AMR activities happening in Africa through the Enhanced Gonococcal Antimicrobial Surveillance Program (EGASP) coordinated by the WHO. Currently, Côte d’ Ivoire, Uganda, Malawi, Zimbabwe and South Africa are involved in this program. Data on the outlook of AMR and supplementary genomic studies that have been conducted since 2023 using data from these countries will be presented. Lastly, the presentation will showcase some pertinent studies on NG that have recently been conducted or are currently being conducted in Africa and highlight their role and contribution to the larger body of literature on NG. For instance, our team in Malawi in collaboration with US collaborators is conducting a study aimed to understand the bacterial and host determinants of the natural history of NG infection outcome and re-infection (genital and extragenital), including asymptomatic infection, and the immunologic responses that are responsible for clearance of infection using a novel longitudinal study of a cohort of individuals with or at risk for Ng infection in Lilongwe, Malawi.
